CONSENT AND GDPR

As part of your treatment, Bowen Whitstable will create and store medical records which may include details concerning medication, treatment and other issues affecting your health conditions. In accordance with the General Data Protection Regulation (GDPR), these records will be retained for eight years (or until 25 years of age in the case of someone aged 16-18) when treatment is ceased in order to comply with legal guidance. These records will be processed in accordance with the 2018 Privacy Notice.
When agreeing to treatment, it is understood that clients will have:

** Completed the health questionnaire in full prior to treatment

** Understand the treatment they are to receive

** Read and understood the above GDPR information and give their explicit consent.

** Agree that appointments cancelled with less than 24 hours notice are chargeable in full
